Thank you for returning to our practice! You have the capability to use our Athenahealth patient portal online to request appointments, reschedule your appointment, check your balance and to make payments. Please find the link on our home page or click here.

Upon arrival to our office, please confirm your demographics (address, telephone #’s, name change, marital status, emergency contact and/or insurance change) or inform us of any other changes. Kindly please arrive at least 15 minutes prior to your appointment. If you have not been seen at our office within the past 3 months, your Digichart must be updated regardless if your history has changed or not.

As a courtesy, you will receive a confirmation call and/or email prior to your appointment date. Specialty co-payments, deductibles & coinsurance payments required by your insurance provider will be collected at the time of your problem visit. We accept cash, check and credit card payments (VISA, MasterCard & Discover). Annual preventive well woman exams without problems require no copayment. Tricare Prime insurance requires a referral for problem visits and prenatal care. A photo Identification card and your insurance card will be requested upon arrival.

We suggest you contact your insurance company for questions regarding your plan and for a copy of your medical formulary if a medication may be prescribed. If you have medical records, bring them to your appointment. If you are referred by your PCP (Primary Care Physician) or another physician for a particular problem, please confirm that we have received your results prior to your appointment.
